Diet Accommodation

Accommodation requests due to food intolerances and/or medical conditions other than disability are evaluated and granted on a case-by-case basis by Santa Fe Public Schools Student Nutrition Department.

To request special meals and/or diet accommodations: please have a physician or recognized medical authority with prescriptive privileges in the state of New Mexico complete and sign the medical statement to request special meals and/or diet accommodations.

Please note if requesting accommodation for a fluid milk substitute due to lactose intolerance, a medical statement is not required.  Instead, please complete the "Parent/Guardian Request for Fluid Milk Substitution” form.

Return completed forms to your school cafeteria or school nurse. Diet orders will remain in effect until parent/guardian submits a formal written request to revoke all or portions of the diet order. For additional questions, please contact our office at 505-467-3601.
